Late summer bloating: the Spleen-dampness pattern I see every August

Every August my schedule fills with the same complaint. Bloating that shows up an hour after lunch. Stools that loosen for no obvious reason. A heavy, foggy feeling after meals that used to sit fine. Patients tell me they haven't changed a thing, and they're usually right. What changed is the season.

Late summer is its own season in Chinese medicine. It maps to the Spleen and Stomach, the digestive pair, and to the climate quality we call dampness. When the air outside is thick and wet and the iced drinks are flowing, the digestive system tends to feel it. This isn't mystical. It lines up with patterns I can measure on labs and stool tests, and it gives me a useful frame for what to actually do.

What late summer does to digestion in TCM terms

In TCM, the Spleen is the organ system that transforms food into usable energy and fluids. It likes warm, cooked, simple food. It dislikes cold, raw, greasy, and sugary. Dampness is what accumulates when that transforming function slows down. Think of a drain that can't keep up with the water coming in.

The classic signs of Spleen dampness read like a gut-health intake form. Bloating and fullness. Loose or sticky stools. Heaviness in the limbs. A thick coating on the tongue. Low appetite paired with sugar cravings. Fatigue that lands right after eating, not before.

Summer makes this worse in two ordinary ways. We eat more cold and raw food, which asks the digestive system to do more warming work. And we drink more sugar, which feeds the wrong microbes downstream. The season doesn't create the weakness. It exposes one that was already there.

The functional-medicine version of the same story

Strip away the language and a lot of Spleen dampness is what I'd call impaired digestion and a disrupted microbiome. The food isn't getting broken down well at the top of the tube, so it ferments further down. Fermentation makes gas. Gas makes bloating and that heavy feeling.

When I run a stool panel on these patients, I often see the same things: low pancreatic enzyme output, signs of maldigested fat or protein, and a microbiome tilted toward gas-producing species. Sometimes there's real small intestinal bacterial overgrowth behind it. Low stomach acid gets more common with age and with chronic stress, and it's strongly linked to bacterial overgrowth. A 2013 meta-analysis in Clinical Gastroenterology and Hepatology found that proton pump inhibitor use, which suppresses acid, was associated with a higher risk of SIBO (Lo WK, Chan WW, 2013). Less acid, more overgrowth. That's the mechanism I'm watching for in the late-summer crowd, especially anyone living on antacids.

For patients who test or present as low, a mealtime betaine HCl can restore acidity enough to get the rest of digestion firing. There's real pharmacology here: a 2013 study in Molecular Pharmaceutics showed betaine HCl reliably re-acidified the stomach in people with drug-induced low acid (Yago et al., 2013). In clinic I pair it with a bitter-forward habit, something as simple as a splash of apple cider vinegar or a few bitter greens before the meal. Saccharomyces boulardii is one I reach for often in this season. It's a beneficial yeast, not a bacterium, so antibiotics don't touch it, and it holds up well against the loose, sticky stools of a damp pattern. A large meta-analysis by Lynne McFarland in World Journal of Gastroenterology (2010) found solid evidence for S. boulardii in several types of diarrhea.
