The order I work the gut in (and why probiotics usually come last)
A patient came in last month convinced her probiotic had stopped working. She'd been on a good multi-strain formula for four months. Bloating by noon, loose stools half the week, and a reaction to foods she used to tolerate fine. She wanted to know which better probiotic to switch to.
She didn't need a better probiotic. She needed to stop starting there.
This is one of the most common patterns I see. Someone reads that the gut is the root of everything, buys the probiotic with the biggest number on the label, and expects the wall to rebuild itself while the fire is still going. The gut heals in a sequence. Skip the early steps and the later ones don't hold.
Late summer is when the gut asks for attention
In Chinese medicine, late summer belongs to the Earth element and the Spleen. The Spleen in TCM isn't the organ your Western anatomy book describes. It's the system that governs transformation and transport: taking food in, pulling usable Qi out of it, and moving the rest along. When Spleen Qi is weak, you get the classic picture. Bloating after meals, fatigue that lands right after you eat, loose stools, a heavy foggy head, and a craving for sweets that never quite satisfies.
I bring this up because the timing matters clinically. Cold drinks, raw salads, and the general looseness of summer eating tax that transform-and-transport function. By August a lot of people are running on a tired digestive system without naming it that way. The seasonal frame and the functional-medicine frame point at the same thing from two directions: your gut lining and your digestive capacity need support before you throw bacteria at the problem.
Step one: quiet the fire and open the exits
Before I repair anything, I want to know what's driving the irritation and whether the waste has somewhere to go.
Active infections, undiagnosed food reactions, low stomach acid, and sluggish bile all keep the gut lining inflamed. Handing someone a barrier-repair nutrient while a parasite or an unaddressed gluten reaction is still present is like patching drywall during a leak. I look at the history first. Antibiotics, travel, food poisoning that things never recovered from, chronic stress, a diet built on the foods that flare them.
Low stomach acid is one I catch constantly and almost nobody tests for. You need adequate acid to break protein down, absorb minerals like zinc and iron, and keep the wrong bugs from setting up shop in the small intestine. Reflux gets treated as too much acid when the picture is often too little, and years on an acid blocker quietly starve the gut of what it needs to defend itself.
Regularity matters here too. If you're not moving your bowels daily, the compounds you're trying to clear get reabsorbed. I'd rather sort out motility, hydration, and bile flow before layering anything on top. This is the same logic behind opening drainage before you bind: the exits have to work first.
Step two: rebuild the wall
Your small intestine is lined by a single layer of cells held together by tight junctions. That one-cell-thick wall decides what gets into your bloodstream and what stays in the gut where it belongs. When those junctions loosen, larger particles slip through and the immune system starts reacting to things it should have ignored. Alessio Fasano's work on zonulin, the protein that regulates those junctions, mapped a lot of how this actually happens (Fasano A. Physiol Rev. 2011;91(1):151-175).
A few nutrients have real evidence behind them for this layer.
L-glutamine is the primary fuel for the cells lining your small intestine, and it helps hold the tight junctions together. Rao and Samak reviewed the mechanism in detail: glutamine supports the assembly and integrity of those junctions, which is exactly the structure that fails in a leaky gut picture (Rao R, Samak G. J Epithel Biol Pharmacol. 2012;5(Suppl 1-M7):47-54). I use it as a foundational piece. In clinic I reach for a clean, single-ingredient L-glutamine from Pure Encapsulations when I want to dose it on its own.
Zinc-carnosine is the other one I lean on. A well-designed study in the journal Gut showed it stabilizes the small-bowel lining and stimulates the gut's own repair processes, and it roughly halved the intestinal damage caused by an NSAID in the same trial (Mahmood A, et al. Gut. 2007;56(2):168-175). Some of my patients do better with a combination formula that pairs glutamine and zinc-carnosine with soothing agents like aloe. A powder like GlutaShield from Ortho Molecular bundles those together, which is convenient when someone won't take four separate things.
This is the step most people skip, and it's the one that determines whether anything else you do sticks.
Step three: reseed, and match the strain to the person
Now the probiotics earn their place. Once the fire is down and the wall is rebuilding, the bacteria you add have a surface worth colonizing.
The mistake I see is treating probiotics as one interchangeable category. They're not. Strains do specific things, and the biggest number on the box isn't the answer.
Saccharomyces boulardii is a good example. It's a beneficial yeast, which is why antibiotics leave it standing. A meta-analysis of 27 trials found it helpful for several diarrhea patterns, including the antibiotic-associated kind (McFarland LV. World J Gastroenterol. 2010;16(18):2202-2222). It's often my first pick for someone whose gut fell apart after a course of antibiotics or a bad travel bug. I keep a straightforward Saccharomyces boulardii on the shelf for exactly that.
For someone who needs broader microbiome support and can tolerate it, I'll go to a spore-based and multi-strain blend. CT-Biotic from CellCore combines spore-forming and non-spore-forming strains, and the spore formers tend to survive stomach acid better than a lot of standard capsules. But I don't lead with the strongest thing. If someone is reactive, a high-count blend can make bloating worse before it makes it better, and then they quit.

Why the order is the whole point
You can do all three of these steps with excellent products and still fail if you run them in the wrong sequence. Reseed a lining that's still inflamed and you're planting in scorched soil. Repair a wall while an infection keeps burning it down and you'll chase your tail for months.
Quiet the fire and open the exits. Rebuild the wall. Then reseed. In my practice that order is what separates the people who get better and stay better from the people who cycle through eight probiotics wondering why nothing holds.
